Public Health Wrap Around Services Coordinator

Review data from database or other electronic format to process requests for wraparound services for Contact or Case.
Direct wraparound service requests to CBO (Community Based Organizations) for assistance or directly facilitate assistance request of Contact or Case.
Correspond with Team Lead and other members of Program for timely response to needs of Contact or Case.
Provide Quarantine and Isolation related correspondence to Contact or Case at the direction of Team Lead or Program Manager.

Skills Required:
3+ years of demonstratable proof of the following skills in a workplace environment.

Proficiency in use of Windows 2016 or newer and Microsoft Office Suite of products for:
Calendars (will be used for links to meetings and trainings)
Zoom (or other video conferencing) this program is used for Program Meetings, Team Meetings and Staff Trainings)
PDF Fillable Forms
Database Entry (multiple systems)
Effective, clear and concise oral and written communication of difficult or complex issues.
Strict adherence to principles of Patient Confidentiality is the number one priority of this Program.
Understanding of the medical terms and principles of: exposure, infection, infectious period, potentially infectious interactions, symptoms of disease, pre-symptomatic and asymptomatic infection for the purposes of communication and wraparound service request review.
Excellent interpersonal communication and culturally sensitive skills in communicating with the public and other Program members, and in the handling of data.

Other Requirements:
A location in the privacy of YOUR HOME where you are able to process electronic information, communicate with Team Lead and other Program members and participate in Zoom meetings without violating confidentiality (e.g., away from others, with the ability to conduct a private conversation)
Personal Computer. Minimum requirements: Windows 10 and Microsoft Office 2016 suite of products.
Ability to work a varied or task-based schedule

Part-time with potential for full-time FIXED SCHEDULE (for purposes of EXAMPLE ONLY: 6hrs per day, 9AM to 3:3OPM, 4 day per week, days TBD). Work is determined by Case volume therefore flexibility is essential.

Preferred but not Required:
Degree in healthcare related field of study. Workplace experience requirement must still be met.
