Insurance Information and Enrollment Documents

The insurance provided to our temporary employees is a low cost limited benefit insurance plan specifically designed to provide you temporary coverage in case of an illness or injury from the time you begin working for us until the time that you are hired on to a permanent position with the job you were placed at. This insurance does not provide comprehensive coverage and certain procedures including but not limited to preventive exams are not covered under this plan and other conditions may have a pre-existing condition exclusion. Please see the information packets by using the links below or by requesting a copy from one of our offices for full details on these limits and exclusions.

Personnel Source staff is unable to answer specific questions about the coverages provided by this insurance. If you have questions about covered procedures or limits that this insurance provides please contact SRC by calling 1-888-772-9682, let them know you are enrolling in benefits provided by Personnel Source under group number 500014.

Please note that new employees have 30 days from the day of hire to enroll. Enrollment after the 30 day new hire window is allowed only if you have a qualifying loss of coverage, family status change or during open enrollment. Please see the information packet if you feel that you may be eligible to enroll due to a loss of coverage or family status change.

**Due to recent changes in law this limited benefits policy is not available to Washington State residents.**

Have you been temporarily laid off?

If so then you may need to make a missed premium payment to SRC to ensure that you do not have a coverage gap in your health insurance.

Personnel Source can only withhold insurance premiums for the weeks in which you receive a paycheck. For any week that you do not work or that you do not receive a check your insurance will show that you have a coverage gap and no claims will be paid for by SRC unless you make a missed premium payment to them.

If you believe that you may have a coverage gap you have 45 days from the beginning of that gap to make a missed premium payment to SRC.

To make a missed premium payments please use a Missed Premium Payment Coupon available in your insurance packet, if you do not have one you can request one from Personnel Source. You can also contact SRC’s Missed Premium and COBRA payment department at 1-888-772-9682.

Direct Deposit

Personnel Source highly encourages all employees to enroll in direct deposit. Not only does this save you time but it also helps to prevent lost or stolen paychecks. Personnel Source can make a direct deposit into any bank account or pre-paid check or credit that has a valid bank routing number.

All changes and enrollments must be done on either Personnel Source's Enrollment Form or on a Direct Deposit Authorization Letter provided by your bank. The form must contain your signature and clearly show your name, account number, routing number, and the name of your bank in order for our staff to process your request.
